Including but not limited to
Re-ablement following a fall, CVA, Surgery, Hospital Admission • Support in transferring from bed to chair, chair to chair or chair to bath, etc, using a hoist or manual handling equipment • Oxygen – needs support to manage • A client may need regular checks to reposition and manage pressure area care • Provide both verbal and visual cues and assistance (full or part) with basic personal care/hygiene, including skin care, bathing, shaving, oral care, toileting, help with dressing/ undressing, hair care • Catheter & Stoma care where the person has more support needs to manage • Under the direction of physio, support with exercise or routine to maintain wellbeing.

A person using the services for the following health conditions
Early to moderate stages of Dementia include Alzheimer’s, Lewy Body, Frontal Lobe, Vascular, etc. • Re-ablement following fall, surgery, hospital admission. • Asthma is generally managed through medication but requires daily monitoring. • Depression and, or anxiety – prone to periods of depression or anxiety SU after a heart attack. • Osteoporosis – requiring daily or regular physical exercises input Parkinson’s. • Insulin-dependent diabetes Type 2 Rheumatoid Arthritis / osteoarthritis Recovery from a stroke • Visual or hearing Impairment (degenerative condition) Living with Cancer • Mild behaviours that may challenge
